Care In The Community

Our Supported Living service gives people the opportunity to live in *a home of their own*, with 24-hour care and support built around their individual needs.

At Personalized Care, we can support individuals and their families to explore and find a suitable home, whether that is a flat, house or bungalow.

The home itself is separate from the care we provide. It is the person’s own home, with their own tenancy and the freedom to make it feel like theirs.

Once the right home is in place, our team provides 24-hour care and support within that home, tailored around the individual.

We support people aged 17 years, when transitioning into adult services, through to 65 years old, and are registered to support people with learning disabilities, autism, physical disabilities and sensory needs.

Our aim is to promote choice and independence through a detailed Person-Centred Plan, taking into account each person’s needs, routines, preferences, communication and what is important to them.

Support can include personal care, medication administration and ordering, mealtimes and dietary needs, mobility, communication, sensory needs, cooking, cleaning, laundry, managing money, attending appointments and accessing the community.

But life at home is about much more than care.

Our teams support people to enjoy their hobbies and interests, spend time with friends and family, try new activities, volunteer, get out and about and become part of their local community.

We are passionate about finding opportunities that are right for the person. That might be something they already love doing or something completely new that they would like to experience.

For people who do not communicate through speech, we take the time to understand their individual way of communicating so they can be involved in choices and decisions about their own life.

Supported Living can also be a positive next step for someone moving from residential care or leaving the family home.

It gives people the opportunity to have a front door of their own, while still having the reassurance of a dedicated team providing support 24 hours a day.

We provide 24-hour support, waking nights, sleep-ins where appropriate and Assistive Technology where required*, based around each person’s assessed needs.
