Supported living 


Our supported living service is a fabulous option for people with a variety of learning disabilities and other associated needs.

We aim to promote choice and independence through a detailed Person Centred Plan. 
Our service will provide this care to individuals aged 17 years old (transitioning into adult services)- 65 years old.

We support people with anything from cooking, cleaning, changing bed linen and doing laundry, to daily personal care such as dressing, washing, taking medication/administration and ordering, Support with managing bills and budgeting, Support with Meal times and Dietary needs, Support with communication needs and sensory impairments – with appointments or help with referrals relating to this ect

A person's support team can also help them pursue hobbies and interests, go out and volunteer, develop new friendships and stay in touch with friends and family.
At Personalized Care we are passionate when seeking out new local activities/opportunities for individuals specific to their own personal likes and interests. We will be promoting self esteem, self worth and self belief alongside of this new journey.

We feel it is important to have a Calendar full of activities because keeping active makes a huge contribution to individuals physical and emotional well-being.Enabling people to develop the confidence and skills to become active members of their community.

Supported living is also a great option for people who are ‘moving away from’ from residential care and who are making the transition to living independently in the community. 

This could be a flat, house or bungalow. People also have the option to share accommodation with others, have friends/family to visit. 
Personalized care will agree a package of care with you and your family, or circle of support, We offer 24-hour support, sleep in's, and waking nights and Assistive Technology if required

 At Personalized Care the service user is always central to the service that we provide. We do not plan for people; we plan with them. We use whatever method is best suited to the individual.
