Safe and Effective Care Services.Personalized Care is a service that provides support for individualswith learning disabilities and other associated conditions – autism spectrum disorder,dementia,mental health conditions (dualdiagnosis) physical needs,sensory impairments,profound and multiple needs and life limiting conditions. Alongside the individual, their family, friends and professionals we will develop a Person Centred Plan to ensure consistency of care.This plan will reflect the individuals specific needs and preferences – Helping us to reduce any behaviours that challenge and improve health and well being. We believe that person specific care that promotes the core values will achieve the best possible outcomes for each service user.At Personalized Care the service user is always central to the service that we provide. We do not plan for people; we plan with them.
Quality Staff
We will promote positive working relationships between the staff team and individual service users. We hope to create a loyal staff team that feel valued and therefore work enthusiastically gaining the best service user outcomes.
Matching new staff with specific service users to achieve positive relationships is very important to us. Individuals that are happy and safe will grow in confidence and progress with all aspects of life. Service users will be encouraged to be a part of the interview process where ever appropriate.
Staff training is to a high standard and very person specific. Personalized Care encourage all it’s staff to reach their full potential.
